FormVu Inc. ("we," "us," or "our") values your privacy. This Privacy Policy explains how we collect, use, and share information when you use our Service.
By using our Service, you agree to our Terms of Service and acknowledge that you have read and understood this Privacy Policy. This Policy applies to all users of our Service, regardless of location.
At the end of this Privacy Policy (9), we've included region-specific supplemental information for users located in: (i) California (pursuant to the California Consumer Privacy Act, as amended by the CPRA); and (ii) other U.S. states with applicable consumer privacy laws, including but not limited to Colorado, Connecticut, Delaware, Florida, Indiana, Iowa, Kentucky, Maryland, Minnesota, Montana, Nebraska, New Hampshire, New Jersey, Oregon, Rhode Island, Tennessee, Texas, Utah, or Virginia, as well as any additional states that enact similar privacy legislation in the future.
1. Definitions
- "Company" refers to FormVu Inc.
- "Contractor" refers to any individual or business that subscribes to and uses FormVu for job estimation and evaluation.
- "Prospect" refers to any individual using FormVu to submit video forms.
- "Subscription" refers to the monthly or annual paid plan contractors purchase to use the Service.
2. Information We Collect
For clarity and ease of understanding, the following information is organized based on whether it is provided by Contractors and/or Prospects, or collected automatically through the use of our Services.
2.1. Information Provided by Contractors
Name, email, company details, payment information (via Stripe), account credentials.
2.2 Information Provided by Prospects
Data and media (e.g., video submissions) submitted through FormVu on behalf of Contractors.
- We process this information on behalf of Contractors, who act as the data controller for Prospect submissions.
2.3 Communications
Support tickets, feedback, emails, or other direct communications with us.
2.4 Account History
Information about your use of the Service, including your trials, subscriptions, discount code usage and related details.
3. How We Use Your Information
When we process your information pursuant to this Privacy Policy, we do so for the following reasons:
3.1 Provisioning of Service
Creating and managing your account and account settings.
3.2 Improving the Service
Improving our Service by learning about and analyzing how you access the Service.
3.3 Processing Payments
Receiving payment from Contractors for paid Service.
- Currently, our payment processor is Stripe. Stripe uses and processes your complete payment information in accordance with its privacy policy.
- We obtain limited information regarding your payment card from Stripe such as the last four digits, country of issuance, and the expiration date.
3.4 Communications
Communicating with Contractors about their accounts, including by sending emails about your transactions and Service-related announcements.
3.5 Complying with the Law
Complying with legal requirements, such as tax and other government regulations and industry standards, contracts, or law enforcement requests.
3.6 Enforcement
Enforcing our Terms of Service, subscription agreements, and other applicable policies. This could include:
- Investigating violations or suspected violations of our terms, policies, or applicable laws
- Taking action in response to misuse of the Service or behavior that may harm us, our users, or third parties
3.7 Protection
Protecting our rights, property, and safety, as well as the rights, property, and safety of our users, employees, service providers, partners, and the public.
4. Data Sharing
We do not sell personal information. However, we disclose information to:
4.1 Submit video forms from Prospects to Contractors
We share personal information, such as video submissions from Prospects, with Contractors to facilitate communication, service inquiries, or project evaluations.
- All such disclosures are made in accordance with applicable laws and are limited to what is necessary to fulfill the intended purpose. We require recipients of this information to use it only for the specified purpose and to protect it in accordance with applicable privacy standards.
4.2 Process payments
We collect data necessary to process your payment if you choose to purchase a subscription to the Service.
4.3 Manage Third-party relationships
We may engage third-party service providers to help deliver, operate, and improve our services.
- These third parties are contractually obligated to protect personal information and only use it as necessary to provide services to us.
4.4 Comply with legal requirements or law enforcement requests
We comply with applicable laws, regulations, legal processes, or governmental requests
- We respond to valid subpoenas, court orders, or other legal obligations.
- We cooperate with law enforcement or other government authorities when required or permitted by law.
- We only disclose data when legally required and challenge unlawful requests.
5. Data Security
We take measures to protect personal information from loss, misuse, unauthorized access, and other unlawful processing, but no system is completely secure, and we can't guarantee absolute security. We cannot promise or guarantee that hackers, cybercriminals, or other unauthorized third parties will not be able to defeat our security and improperly collect, access, steal, or modify your information

We retain personal information only for as long as necessary to provide our Service, fulfill the purposes outlined in this Privacy Policy, comply with our legal obligations, resolve disputes, and enforce our agreements.

Contractors may request access, correction, or deletion of their data (or that of their Prospects) by contacting us at https://www.formvu.com/contact.
- Please note that we may need to verify your identity in connection with your request(s). If you do not have access to your account, we may require you to provide us with additional information to verify your identity.
- As permitted by law, we may retain personal information to comply with legal requirements, protect rights, resolve disputes, and enforce our terms and policies.
If you are a Prospect, you should contact the applicable Contractor to exercise your rights regarding any information they hold about you because the Contractor is the controller of that data.
8. Changes to This Policy
When we update our Privacy Policy, we'll revise the "Effective Date" at the top and post the updated version on our site. For significant changes, we'll provide notice in advance—either prominently on our site or directly to Contractors.
We encourage you to review the Privacy Policy periodically to stay informed about how we handle your information and your relationship with us.
9. Additional Information for United States Residents with Specific Privacy Rights
If you are a resident of California, Colorado, Connecticut, Delaware, Florida, Indiana, Iowa, Kentucky, Maryland, Minnesota, Montana, Nebraska, New Hampshire, New Jersey, Oregon, Rhode Island, Tennessee, Texas, Utah, or Virginia (as well as any additional states that enact similar privacy legislation in the future), you may have certain rights under applicable privacy laws. These may include the right to:

You also have the right to not be discriminated against (as provided for in applicable law) for exercising your rights. We do not discriminate against residents who choose to exercise these rights.
